Bed Hold Deposit Agreement

I, (First name Last name), understand and agree to the following:

A $200 bed hold deposit is required to guarantee placement at Sober Living in Delray prior to your intake and will be applied toward intake fees at admission to cover the following fees:

$100 Admin Fee
$100 Security Deposit (refundable after completing 6 month commitment)

This bed hold deposit is non-refundable under any circumstances, including but not limited to:

Deciding not to move in after reserving a bed.
Leaving your treatment program prior to your intake (AMA)
Being discharged from your treatment program
Moving into Sober Living in Delray and choosing not to stay

**A deposit secures your placement prior to admission. While not required, it is strongly recommended, as available beds are assigned on a first-come, first-serve basis, with priority given to those who have placed a deposit. If you choose to not place a deposit we cannot guarantee a bed will be available until 24hrs prior to your intake date.
